Please find in the mailer:
- Two
boxes labeled A and B each containing two pouches and
sterile swabs with their respective culture tubes.
- Sample
collection and shipping instructions.
- Antibacterial
sanitizer gel packs.
- Requisition
form.
- Payment
form.
- Plastic
leak-proof biohazard bag.
- White
padded envelope.
- FedEx
mailer with prepaid label.
Shipping Instructions:
Please follow the mailing instructions
carefully.
1.
After obtaining both urethral (A) and semen (B)
specimens aseptically, place both white boxes into the
plastic leak-proof biohazard bag.
2.
Place plastic leak-proof biohazard bag, payment, and
requisition form into the white padded envelope.
3.
Insert the white padded envelope into the FedEx
mailer.
4.
Mail via FedEx overnight.
The cost to perform a semen
and urethral culture, testing for both aerobes and anaerobes, is $225.
This includes the sample collection kit and overnight shipping cost.
